Vacation Bible School Parent/Guardian Waiver & Release
 

By registering my child for Finish the Race Vacation Bible School at Servants Church, I acknowledge and agree to the following:

 

Participation

I understand that my child will participate in a variety of activities, including games, crafts, recreation, outdoor activities, and other age-appropriate programming. I understand that participation involves normal risks associated with children's activities.

 

Medical Authorization

In the event of an illness or injury, I authorize Servants Church volunteers and staff to obtain appropriate medical treatment for my child if I cannot be reached immediately. I understand that every reasonable effort will be made to contact me or my emergency contact before treatment is authorized whenever possible.

I accept responsibility for any medical expenses incurred on behalf of my child.

 

Liability Release

I voluntarily release and hold harmless Servants Church, its pastors, employees, volunteers, and representatives from liability for injuries, losses, or damages arising from my child's participation, except in cases of gross negligence or willful misconduct.

 

Wet Zone Activities (Friday Only)

I understand that Friday's activities include water games and a designated Wet Zone. I acknowledge that my child may become wet during these activities.

 

I understand it is my responsibility to send my child with:

• A backpack

• A change of clothing

• A towel

 

Private restrooms will be available for children who wish to change into dry clothes. Children may also wear water-friendly clothing that dries quickly if they choose.

Photo & Video Permission

I grant Servants Church permission to photograph or record my child during VBS activities for use in church publications, social media, promotional materials, and the church website.

If I do not wish for my child to appear in photos or videos, I understand it is my responsibility to notify Servants Church in writing before the event.

 

Behavior

I understand that respectful behavior is expected from all participants. Servants Church reserves the right to dismiss any participant whose behavior places themselves or others at risk or significantly disrupts the program. Every reasonable effort will be made to contact a parent or guardian before dismissal.

 

Acknowledgment

I certify that I am the parent or legal guardian of the registered child and have read, understood, and agree to this waiver and release.

 

By submitting my child's registration, I acknowledge and accept these terms.
